Welcome to the Walnut Creek CERT Field Team Message Form trainer. The purpose of this trainer is to familiarize you with the form you will be using to report to your incident command post. You will start with a short registration section.
This is the field team message form in its entirety. The forms used in a real disaster at the command post is a three-part form for quick distribution to the proper sections. The field (SAR) teams will utilize a single copy form. Your cache should have a supply of forms and it wouldn’t hurt to keep some in your backpack. Except for the Header and Footer sections, each section is optional and only need to be completed if it relates to your incident.
